How can we tell between having natural human emotions and potentially going into a mania or depression (especially with other co-morbid conditions that effect emotional extremes)?

Myself when I had strange ideas like I was the ruler of the world or when I had the impression that police was after me which was not true, these are all symptoms, things that are not real, you can be aware of them through talking to others as they will see what is real and what is not so you know where you are.
Good question, I ask myself and therapist every day. What I have been trying is CBT, DBT, distraction and doing things on the pleasant activities list on the web.
