You have to make an appointment with your primary care doctor and tell them that you want evaluated. Some primary care doctors will prescribe through themselves, but others will send a referral out to the specialty that they think is necessary.

ADHD isn't something you can fully self-diagnose, but signs can include trouble focusing, getting distracted easily, or feeling restless. It's best to speak with a professional for a proper evaluation. I had a good experience talking to someone at Phoenix Rebellion Therapy.
