Located in Pine Bluff, AR, the ACT Chronic Heart Failure Program is dedicated to providing comprehensive care, education, and support to individuals living with chronic heart failure. Our program is designed around three key elements: Assess, Control, and Thrive. We start by assessing each patient’s unique situation, offering essential education on lifestyle changes, medication management, and nutrition tailored to individual needs. Participants then learn to manage their condition actively through exercise classes at our CHF Outpatient Therapy Center, promoting physical fitness three times a week. Finally, those who complete the program are encouraged to continue their wellness journey with access to the Wellness Center, where joining fees are waived. Understanding the challenges faced by uninsured and underinsured members of our community, we are committed to offering fair billing options that accommodate individual financial situations, ensuring everyone has access to vital medical care and support.
