The ACT Chronic Heart Failure Program in White Hall, AR, is dedicated to empowering individuals living with chronic heart conditions through compassionate care, education, and support. Our comprehensive program consists of three key components: Assess, Control, and Thrive. We begin by assessing each patient’s capacity to engage in our program, providing crucial education on lifestyle changes, medication management, and nutrition tailored to their needs. Participants will then learn to manage their condition effectively through structured physical fitness classes held three times weekly at our CHF Outpatient Therapy Center. The program culminates with the opportunity to thrive by graduating into our Wellness Center, where joining fees are waived. We understand that many in our community may face challenges due to being uninsured or underinsured; therefore, JRMC is wholeheartedly committed to offering accessible healthcare options that align with individuals' financial means, ensuring that everyone has the chance to receive the care they deserve.
