Nutrition for Longevity (N4L) is dedicated to enhancing the health and well-being of our community in Crestview, FL, by providing Home Delivered Meals and Medically Tailored Meals for those eligible under Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS), or the Older Americans Act. Our meal delivery program focuses on individualized nutrition, with meals crafted by a Registered Dietitian to address a wide range of medical needs, including Chronic Kidney Disease and Congestive Heart Failure, while ensuring meals are free from dairy, peanuts, gluten, and pork. Whether you qualify through government assistance or opt for our self-pay program at $7.75 per meal, we are committed to ensuring access to nutritious food that fits each client's dietary requirements. For assistance with eligibility, we encourage clients to connect with their Case Manager, Medicaid provider, or the local Area Agency on Aging. We also offer Medical Nutrition Therapy and Nutrition Counseling to further support our clients' health goals.
