Our mission is to deliver coordinated care that encompasses primary medical services, specialized referrals, laboratory and diagnostic testing, and essential pharmaceuticals. Additionally, we offer vital support through mental health, nutritional, and limited dental services, alongside substance abuse assistance and personalized medical case management. If you are residing in Okaloosa or Walton Counties and are HIV positive, we invite you to complete our eligibility process to access these critical services, which may be funded fully or partially through the Ryan White program.
