Located in Hiram, GA, the HIV Elimination Program is dedicated to providing essential HIV care and support to low-income individuals living with HIV, particularly those who are uninsured and underserved. Our comprehensive approach includes HIV medical case management, medical nutrition therapy, mental health and dental services, outpatient substance abuse treatment, and legal advocacy. We are committed to ensuring that vulnerable populations receive the critical medical care they need, along with supportive services that enhance their quality of life. Importantly, we also address the nutritional needs of our clients by offering food assistance, recognizing that access to sufficient and nutritious food is a vital component of their overall health and well-being.
