Located in Jesup, GA, Diversity Health Center is dedicated to providing comprehensive healthcare services that prioritize the well-being of each individual. Our mission is to support all residents of Georgia, especially those in families earning at or below 200% of the federal poverty level. We offer a wide range of services including primary healthcare for children and adults, immunizations, wellness exams, women's health, family planning, chronic disease management, case management, and access to discounted medications. Additionally, we provide education on health, nutrition, and parenting, along with essential services such as free HIV testing, dental, vision, and podiatry care. For added convenience, transportation services are available upon request. We operate on a sliding fee scale to ensure affordability, while accepting both Medicaid and Medicare to assist our community effectively.
