Located in Suwanee, GA, the HIV Elimination Program is dedicated to improving the lives of low-income individuals living with HIV who are uninsured and underserved. Our mission is to ensure access to critical services for those who might not receive the medical care they need. We provide a holistic approach through HIV medical case management, medical nutrition therapy, mental health support, dental services, outpatient substance abuse treatment, and legal advocacy. To meet the basic nutritional needs of our clients, we also offer food assistance, empowering individuals to lead healthier lives while overcoming barriers to healthcare.
