Located in Palatine, IL, the Maternal Child Health (MCH) Case Management Program is dedicated to empowering pregnant women and families with infants up to one year old. Our compassionate case managers provide vital support, recognizing that you are your baby's first teacher and advocate. We offer essential services, including pre- and post-natal care, healthcare referrals, assistance with KidCare and FamilyCare insurance, and guidance on immunizations and well-child visits. In addition, we conduct personalized care plans and home visits to help you achieve your goals. We also connect families with resources such as WIC appointments, food stamps, emergency diapers, formula, and food assistance from Firman’s Food Pantry. Our community engagement includes quarterly support groups, baby showers, and free clothing give-aways, along with an emergency fund program to support clients facing urgent financial needs.
