Located in Fort Riley, KS, the Living with Type 2 Diabetes Program is a free, accessible initiative designed for individuals newly diagnosed with type 2 diabetes. Over a 12-month period, this comprehensive program guides participants through essential diabetes education and management, providing six e-booklets tailored to meet them wherever they are on their health journey. The program also features a monthly e-newsletter filled with seasonal tips and resources available in both English and Spanish, along with information on local community events. Enrollees who opt to share their phone number can participate in the monthly Ask the Experts Q&A series, an opportunity to engage directly with ADA experts, ask pressing diabetes-related questions, and learn vital information about the connection between diabetes and heart health.
