This program, available in both English and Spanish, includes six informative e-booklets that focus on essential diabetes management topics tailored to participants' individual needs. Each month, enrollees receive a helpful e-newsletter featuring seasonal tips, valuable resources, and updates about local community events. Participants have the opportunity to engage in a monthly Q&A series with experts from the American Diabetes Association, where they can submit diabetes-related questions, gain insights into the connection between diabetes and heart health, and enjoy giveaways that support their journey toward better health and well-being.
