Diabetes HELP is dedicated to enhancing the health and well-being of Louisiana Medicaid recipients living with diabetes in Denham Springs. Our program prioritizes quality education and support through a comprehensive diabetes self-management education initiative, fostering collaboration among patients, pharmacists, physicians, and the broader healthcare team. Participants engage in small group sessions where they learn essential diabetes self-management skills and interact with licensed pharmacists, creating a supportive community. Our educational sessions cover vital topics including an overview of diabetes, coping strategies, nutrition and diet, physical activity, blood sugar monitoring, long-term complications, and medication management. While we emphasize patient education, we complement, rather than replace, regular physician care. Developed within the framework of the Diabetes Initiative by the Robert Wood Johnson Foundation, Diabetes HELP is committed to empowering individuals to take charge of their health and manage their condition effectively.
