Diabetes HELP is dedicated to enhancing the health and well-being of Louisiana Medicaid recipients living with diabetes in Duson, LA. Our program offers comprehensive diabetes self-management education that empowers participants through quality instruction and collaborative care among healthcare providers, pharmacists, and the community. In small group settings, individuals learn valuable skills to manage their condition, including nutrition, physical activity, and coping strategies, while also having the chance to connect with licensed pharmacists for medication guidance. Our educational sessions cover crucial topics such as managing blood sugar levels, recognizing symptoms, and understanding long-term health implications. Although we complement traditional physician care, our focus is on enabling participants to take charge of their health through informed self-management strategies, utilizing best practices from the Diabetes Initiative framework.
