Diabetes HELP, located in Farmerville, LA, is dedicated to enhancing the health and wellbeing of Louisiana Medicaid recipients living with diabetes. Our program offers comprehensive diabetes self-management education that empowers participants to take control of their condition through informative sessions in nurturing small group environments. By fostering collaboration among patients, pharmacists, physicians, and other healthcare professionals, we create a supportive network to effectively manage diabetes. Participants learn vital skills covering topics such as nutrition, physical activity, coping strategies, medication management, and recognizing critical symptoms, all of which are crucial for maintaining day-to-day health.
