Based in Gonzales, LA, the Diabetes HELP program is dedicated to enhancing the health and quality of life for Louisiana Medicaid recipients living with diabetes. Through our comprehensive diabetes self-management education program, we equip participants with essential skills and knowledge in a supportive small group environment. Our collaborative approach brings together patients, pharmacists, physicians, and healthcare professionals, fostering a community of care and understanding.
