Diabetes HELP is dedicated to enhancing the health and well-being of Louisiana Medicaid recipients living with diabetes in Homer, LA. Our program provides quality education and support through a comprehensive diabetes self-management education initiative, fostering collaboration among patients, pharmacists, physicians, and healthcare providers. Participants engage in small group sessions where they learn essential diabetes management skills and have the opportunity to discuss their experiences and challenges with licensed pharmacists and peers. While not a substitute for regular medical care, our educational offerings equip individuals with knowledge in critical areas such as nutrition, physical activity, recognizing symptoms of blood sugar fluctuations, long-term health risks, and medication management.
