At Diabetes HELP in Leesville, LA, we are dedicated to enhancing the health and quality of life for Louisiana Medicaid recipients living with diabetes. Our mission revolves around providing exceptional diabetes self-management education and fostering a collaborative care approach among patients, pharmacists, physicians, and other healthcare professionals. Through small group sessions, participants learn essential self-management skills while engaging in meaningful discussions about their condition. Our program covers a variety of crucial topics, including nutrition and diet, coping strategies, physical activity, and recognizing signs of blood sugar fluctuations. While we complement existing physician care, our focus is on empowering individuals with the knowledge to effectively manage their diabetes and prevent long-term complications. Our framework, developed in partnership with the national Diabetes Initiative by the Robert Wood Johnson Foundation, underscores our commitment to supporting each participant's journey towards better health.
