The Living with Type 2 Diabetes Program in Leesville, LA is a supportive, free initiative designed for individuals recently diagnosed with type 2 diabetes. Over the span of 12 months, participants can access valuable resources in both English and Spanish, including six informative e-booklets tailored to their unique needs post-diagnosis. Our monthly e-newsletters provide seasonal tips, community event information, and additional resources to help individuals navigate their journey. Enrollees who share their phone number are invited to join our monthly 'Ask the Experts' Q&A series, where they can engage with American Diabetes Association experts, inquire about diabetes management and its connection to heart health, and participate in conversations that foster understanding and resilience. Together, we strive to empower individuals to live well with diabetes and actively engage with their health.
