Diabetes HELP, located in Livingston, LA, is dedicated to enhancing the health and quality of life for Louisiana Medicaid recipients living with diabetes. Our mission is straightforward: to provide comprehensive diabetes self-management education and support to empower individuals managing this condition. Through engaging group sessions, participants receive essential education on diabetes management, covering vital topics such as nutrition, physical activity, coping skills, and medication reviews. We foster a collaborative care environment that includes licensed pharmacists and healthcare professionals, ensuring that participants not only learn effective self-management techniques but also have the opportunity to share their experiences and challenges with peers. While our program complements regular physician care, our focus is on equipping individuals with the knowledge and skills necessary to navigate their diabetes confidently and effectively.
