Located in Mansfield, LA, the Diabetes HELP program is dedicated to enhancing the health and quality of life of Louisiana Medicaid recipients living with diabetes. Our mission centers around empowering participants through comprehensive diabetes self-management education, fostering collaboration among patients, pharmacists, physicians, and healthcare teams. In small group settings, we provide essential skills and support, enabling individuals to effectively manage their condition. With a focus on practical topics such as nutrition, physical activity, coping mechanisms, and medication management, our educational sessions equip participants to engage confidently in their care alongside their regular healthcare providers. Developed with insights from the Diabetes Initiative, our program is designed to nurture self-management and improve overall diabetes care in the community.
