Diabetes HELP is dedicated to enhancing the health and well-being of Louisiana Medicaid recipients living with diabetes through comprehensive education and support. Based in Natchitoches, LA, our program focuses on empowering participants with essential diabetes self-management skills in a nurturing small group environment. We foster collaboration among patients, pharmacists, physicians, and other healthcare professionals to ensure well-rounded care. Our educational sessions address vital topics including diabetes basics, coping skills, nutrition and diet, physical activity, recognizing blood sugar fluctuations, managing long-term complications, and medication review. While the Diabetes HELP program complements regular physician care, it emphasizes the importance of patient education and self-management, drawing on the proven framework developed by the Diabetes Initiative of the Robert Wood Johnson Foundation to effectively support our community.
