Diabetes HELP, located in Oakdale, LA, is dedicated to enhancing the health and quality of life for Louisiana Medicaid recipients dealing with diabetes through comprehensive diabetes self-management education. Our program prioritizes practical education and support, helping participants develop essential self-management skills in a collaborative and encouraging group setting. We empower individuals by providing opportunities to engage in discussions with licensed pharmacists and other members of the health care team. Our educational sessions encompass critical topics such as understanding diabetes, nutrition and diet, coping strategies, physical activity, and recognizing the signs of fluctuating blood sugar levels, while also reviewing medications to ensure effective management of the disease. While we complement regular physician care, our mission is to equip participants with the knowledge and skills necessary for successful self-management, founded on the framework of the Diabetes Initiative by the Robert Wood Johnson Foundation.
