Located in Rayne, LA, the Diabetes HELP program is dedicated to enhancing the health and well-being of Louisiana Medicaid recipients living with diabetes. Our mission is to empower individuals through comprehensive diabetes self-management education, fostering effective collaboration among patients, pharmacists, physicians, and healthcare team members. We offer small group sessions where participants gain essential skills to manage their diabetes, including insights on nutrition, physical activity, and medication review. Our program does not replace regular physician care but serves as a vital resource for education on managing diabetes and preventing long-term complications. Utilizing the proven framework of the Diabetes Initiative from the Robert Wood Johnson Foundation, we focus on supporting self-management to improve quality of life for our participants.
