Based in Rayville, LA, the Diabetes HELP program is dedicated to enhancing the health and well-being of Louisiana Medicaid recipients living with diabetes. Our mission is to empower individuals through comprehensive diabetes self-management education and support, fostering effective collaboration among patients, pharmacists, physicians, and healthcare providers. In our engaging small group sessions, participants gain essential skills to manage their diabetes, including coping strategies, nutrition guidance, physical activity recommendations, and understanding the importance of medication adherence.
