Diabetes HELP is dedicated to enhancing the health and quality of life for Louisiana Medicaid recipients living with diabetes in Sulphur, LA, through comprehensive diabetes self-management education and support. Our program provides a welcoming environment where participants can learn vital diabetes self-management skills in small group settings, fostering community and collaboration. We facilitate meaningful discussions with licensed pharmacists and fellow participants to empower individuals in managing their condition effectively. While we do not replace regular physician care, our focus is on providing essential education, covering topics such as nutrition, physical activity, medication review, and coping strategies. Utilizing the framework established by the Diabetes Initiative from the Robert Wood Johnson Foundation, we aim to promote self-management and improve the overall health outcomes for those facing diabetes challenges in our community.
