Diabetes HELP is dedicated to enhancing the health and quality of life for Louisiana Medicaid recipients living with diabetes in Thibodaux, LA. Our program offers comprehensive diabetes self-management education in supportive small group settings, fostering a collaborative environment among patients, pharmacists, physicians, and health care professionals. Participants gain essential skills to manage their diabetes effectively while engaging in meaningful discussions about their health. Our educational sessions cover key topics such as understanding diabetes, developing coping strategies, nutrition and diet planning, physical activity recommendations, recognizing signs of altered blood sugar, and long-term health considerations. While we do not replace regular medical care, our aim is to empower individuals with the knowledge and support needed to take charge of their diabetes management, all built on a foundation created by the Diabetes Initiative from the Robert Wood Johnson Foundation.
