Located in Burtonsville, MD, Diabetes and Heart Health Services is dedicated to empowering individuals with the education and support necessary to prevent and manage diabetes and heart disease. With a specific focus on the African American community, where 33% of those with diabetes also face heart disease, our program offers essential services including Chronic Disease Management Classes, self-management counseling, and health screenings. We actively engage with the community by providing valuable resources and information at various events across Montgomery County, including health fairs, walks, and presentations. Funded by the Montgomery County Department of Health and Human Services and implemented by McFarland & Associates, Inc., we are committed to fostering healthier lives through awareness and education.
