At the THRIVE Program in Chestertown, MD, we are dedicated to improving the lives of individuals affected by Hepatitis C and HIV through comprehensive prevention, treatment, and management services. Our offerings include outpatient health services for general and specialized HIV care, medical case management both at our clinic and in the community, mental health support, and substance use treatment services, including Suboxone® for opioid use disorder. We also provide essential health resources such as nutritional therapy, a food bank, and home-delivered meals to ensure our clients have access to proper nutrition. Additionally, we assist with transportation needs and offer financial help for health insurance premiums and emergency medications. Our mission is to empower individuals and remove barriers to health and wellness, making a meaningful difference in our community.
