We provide comprehensive outpatient health services, including primary and specialty care, tailored medical case management, and essential mental health services. Our commitment extends beyond medical care, offering outpatient treatment for substance use disorders like opioid dependency, complete with Suboxone® prescriptions. Additionally, we support overall well-being through medical nutritional therapy, which includes consultations with nutritionists and home-delivered meals. The THRIVE Program is also here to assist with legal services, provide transportation support, help with health insurance premiums, and offer emergency financial assistance for medications.
