Located in Havre De Grace, MD, the THRIVE Program is dedicated to the prevention, treatment, and management of critical health issues such as Hepatitis C and HIV. We offer comprehensive outpatient health services, including primary care and specialized HIV care, along with accessible medical case management in both clinical and community settings. Our holistic approach also encompasses mental health support, outpatient substance use treatment, and medical nutritional therapy, which includes consultations with registered nutritionists. To ensure our clients have the resources they need, we provide food assistance through our food bank and home-delivered meal services, transportation support, health insurance premium assistance, and emergency financial aid for medications. At the THRIVE Program, we prioritize the well-being of our community by addressing health challenges with compassion and practical resources.
