Living Well with Chronic Disease in Jessup, MD, offers a vital, free 6-week self-management program designed to empower individuals in managing chronic health conditions. Our program combines education and practical skills to help participants cope with challenges such as frustration, fatigue, chronic pain, and feelings of isolation. We focus on fostering effective communication with family, friends, and health professionals, while also teaching exercises to improve strength, flexibility, and endurance. Participants will learn how to enhance their nutrition and cultivate healthy eating habits. Each individual will receive the Living Well Toolkit, which includes self-paced materials and activities to support their journey. Weekly, participants will engage in a small group conference call, fostering community and sharing progress with an instructor to enhance their self-management skills.
