Diabetes and Heart Health Services in Kensington, MD, is dedicated to empowering individuals to prevent and manage diabetes and heart disease through comprehensive education and support. Recognizing the strong link between these conditions, particularly within the African American community where 33% of those with diabetes also face heart disease, our program offers a range of essential services. We conduct Chronic Disease Management Classes, provide self-management counseling, and facilitate health screenings to promote better health outcomes. Additionally, we actively share valuable resources and information at community events throughout Montgomery County, including walks, health fairs, and presentations. Funded by the Montgomery County Department of Health and Human Services and implemented by McFarland & Associates, Inc., we are committed to fostering a healthier community.
