Cancer Transitions, located in Potomac, MD, is dedicated to empowering cancer survivors through a comprehensive, evidence-based program designed for individuals who have completed treatment within the last 24 months, although all survivors are welcome to participate at any stage of their journey. Our mission is to support every survivor with essential resources that include nutritional guidance, tailored exercise plans, emotional well-being education, and medical management assistance.
