Located in Silver Spring, MD, Diabetes and Heart Health Services is dedicated to empowering individuals with the knowledge and resources necessary to prevent and manage diabetes and heart disease, two critical health issues that often intersect, particularly among African Americans, a population in which 33% living with diabetes also battle heart disease. Our comprehensive program offers a variety of services, including Chronic Disease Management Classes, self-management counseling, and health screenings, all aimed at fostering better health outcomes. We actively participate in community outreach by providing information at local events throughout Montgomery County, such as health fairs and wellness presentations, ensuring that our message of prevention and health awareness reaches those who need it most. Funded by the Montgomery County DHHS and implemented in collaboration with McFarland & Associates, Inc., we are committed to improving the health and well-being of our community.
