The Living with Type 2 Diabetes Program in Coldwater, MS, is designed to support individuals recently diagnosed with type 2 diabetes by offering a comprehensive, free, and accessible approach to diabetes education and management over a span of 12 months. Participants will receive six informative e-booklets tailored to their specific needs and a monthly e-newsletter featuring seasonal tips and local community event information, available in both English and Spanish. Enrollees who provide their phone number can also join our monthly 'Ask the Experts' Q&A series, where they can engage with diabetes specialists, gain insights on the connection between diabetes and heart health, and receive helpful resources and giveaways, empowering them to thrive while managing their condition.
