Home & Community Based Services, located in Plains, MT, is dedicated to empowering older adults and individuals with disabilities to thrive in their own communities. Our compassionate team of Social Work Case Managers and Registered Nurses collaborates with members to create personalized service plans that focus on their unique strengths and needs. We offer a range of vital services, including case management, social outings, homemaking, respite care, personal emergency response systems, and private duty nursing. Our program also provides delivered meals, transportation, home and vehicle modifications, life coaching, assisted living support, pain and symptom management, adaptive recreational therapy, senior companionship, and access to specialized medical equipment and supplies.
