Cancer Transitions is a compassionate program located in Kannapolis, NC, designed to empower cancer survivors who have recently completed treatment. While primarily focused on individuals who have finished treatment within the last 24 months, our resources are beneficial for survivors at any stage in their recovery journey. We offer a comprehensive range of services including nutritional support, personalized exercise plans, emotional health education, and medical management education. Our mission is to help survivors thrive by providing the tools and support necessary for a healthy, fulfilling life after cancer.
