Our interdisciplinary team is committed to ensuring that every participant's needs are met, offering medical care, rehabilitation therapy, transportation, recreational activities, social services, nutrition planning, in-home support, and spiritual care. We aim to create a supportive environment where older adults can thrive. For those eligible for both Medicare and Medicaid or Medicaid alone, there are no out-of-pocket costs, ensuring accessibility to our vital programs. Participants with Medicare who do not qualify for Medicaid will incur a monthly fee, while those without Medicare or Medicaid will also have a monthly fee. We are here to provide practical and compassionate support to our community’s seniors.
