Project Transition provides specialized residential treatment for adults aged 18-65 with mental health conditions, including those with co-occurring substance use disorders, intellectual development disorders, or physical health challenges. Their unique approach centers on creating a therapeutic community within apartment settings at Cypress Grove Apartments, where participants are called "members" rather than clients or patients to foster a sense of belonging and community integration. The program employs dialectical behavior therapy (DBT) with a strength-based, individualized focus. A dedicated team of approximately 30 professionals, including certified peer and recovery specialists, provides support daily from 7 a.m. to 9 p.m., with emergency assistance available after hours. The structured weekly schedule includes skills workshops, medication management, individual therapy sessions, psychiatric appointments, group therapy, community meetings, goal planning, and access to Alcoholics Anonymous (AA) and Narcotics Anonymous (NA) meetings. Transportation is provided for off-site meetings and medical appointments. Project Transition also offers systems of support (SOS) for children and adults whose behavioral health concerns affect their community functioning, working collaboratively with crisis teams and family support systems to reduce hospitalizations and prevent crises. The program measures success through 13 specific criteria and allows daytime family visits with housemate approval. Members reside in partially unfurnished apartments where they can bring personal belongings, supporting their journey toward meaningful, community-integrated lives while reducing stigma and promoting normalized social expectations.
