Cancer Transitions is a dedicated program in Rochester, NY, designed to support cancer survivors who have recently completed their treatment. While our primary focus is on individuals within the last 24 months of their survivorship journey, we extend our resources to benefit those further along their path as well. Our evidence-based approach includes vital nutritional support, customized exercise plans, educational resources on emotional health, and guidance on medical management.
