Located in Beachwood, OH, Nutrition for Longevity (N4L) is dedicated to enhancing health and well-being through our Home Delivered Meals and Medically Tailored Meals programs. We cater to eligible clients enrolled in Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS) programs, and the Older Americans Act, as well as individuals seeking a self-pay option for meal delivery. Our meals are meticulously planned by a Registered Dietitian to meet the specific nutrient requirements for a range of health conditions, including Chronic Kidney Disease and Congestive Heart Failure, ensuring that they are both nourishing and tailored to our clients' needs. With a focus on dietary restrictions, our meals are free from dairy, peanuts, gluten, and pork. For those enrolled in Medicaid or the Older Americans Act, we're here to help you determine eligibility for meal delivery; reach out to your Case Manager or local Area Agency on Aging for assistance. Medicare Advantage beneficiaries can check their plan details, as some offer meal support as part of chronic care. For individuals not covered by these programs, our self-pay option allows for personalized meal selections at an affordable price of $7.75 per meal. Additionally, we provide Medical Nutrition Therapy and Nutrition Counseling to further support our clients' health journeys.
