Cancer Transitions is a dedicated program located in Chepachet, RI, designed to support cancer survivors who have finished their treatment. While primarily tailored for those who have completed treatment within the last 24 months, our services are beneficial for survivors at any stage of their journey. We offer comprehensive resources including nutritional support to help maintain a healthy diet, customized exercise plans to enhance physical wellbeing, emotional health education for mental resilience, and medical management guidance to navigate post-treatment care. Our mission is to empower survivors with the knowledge and support they need to thrive in their survivorship.
