Located in Harmony, RI, the Community Health Network Program, part of the Rhode Island Parent Information Network (RIPIN), is dedicated to empowering individuals to take charge of their health through a comprehensive and centralized referral system. We connect clients with evidence-based health programs, offering vital services including system navigation and referrals to specialized programs. Our focus extends to a range of health conditions such as diabetes, asthma, and heart disease, and we provide health education alongside free or low-cost classes and workshops.
