Health Coaches for Hypertension Control (HCHC) in Greenville, SC, is dedicated to empowering adults living with hypertension through an evidence-based chronic disease self-management program. Our trained Health Extension Agents, acting as supportive Health Coaches, guide participants through eight in-person educational sessions that adhere to the guidelines set by the Joint National Committee 7 (JNC 7).
