Health Coaches for Hypertension Control (HCHC) in Greer, SC, is dedicated to empowering individuals with hypertension to take charge of their health through a structured, evidence-based self-management program. Our specially trained Health Extension Agents serve as supportive Health Coaches, guiding participants through eight in-person educational sessions that adhere to the Joint National Committee 7 (JNC 7) guidelines.
