Health Coaches for Hypertension Control (HCHC) in Mauldin, SC, is dedicated to empowering adults living with hypertension through an evidence-based chronic disease self-management program. Our specially trained Health Coaches provide valuable support and education in a welcoming environment, offering eight in-person sessions that align with the guidelines of the Joint National Committee 7 (JNC 7). Participants engage in comprehensive discussions on crucial topics such as understanding hypertension, nutritious dietary choices, physical activity, and effective stress management.
