Health Coaches for Hypertension Control (HCHC) is dedicated to empowering adults in Piedmont, SC, to effectively manage hypertension through a structured and evidence-based chronic disease self-management program. Our specially trained Health Extension Agents, acting as Health Coaches, provide comprehensive support across 8 in-person educational sessions designed to promote a healthy lifestyle in line with the Joint National Committee 7 (JNC 7) guidelines. Participants engage in vital topics, including understanding hypertension, nutrition tailored to lower blood pressure, appropriate physical activity, and effective stress management techniques.
