Health Coaches for Hypertension Control (HCHC) in Taylors, SC is dedicated to empowering adults living with hypertension through a comprehensive, evidence-based chronic disease self-management program. Our specially trained Health Extension Agents, acting as skilled Health Coaches, provide participants with eight engaging in-person educational sessions based on the guidelines of the Joint National Committee 7 (JNC 7). Each session covers vital topics such as understanding hypertension, nutrition specifically for managing high blood pressure, the importance of physical activity, and effective stress management techniques. By fostering a supportive environment, we aim to inspire healthier lifestyles and enhance the well-being of our community.
