Health Coaches for Hypertension Control (HCHC) is a dedicated program located in Travelers Rest, SC, focused on empowering adults to manage hypertension effectively. By utilizing specially trained Health Extension Agents as Health Coaches, we offer an evidence-based chronic disease self-management experience that includes eight in-person educational sessions. Our comprehensive curriculum adheres to the guidelines set by the Joint National Committee 7 (JNC 7) and covers essential topics such as understanding hypertension, nutritious dietary practices, the importance of physical activity, and effective stress management. We are committed to promoting a healthier lifestyle in our community, ensuring that participants receive the support and knowledge necessary to take charge of their health with confidence.
